Job Description
Directs, plans, organizes, and evaluates the Social Work staff, daily operations, services and programs. Oversees comprehensive, culturally competent, psychosocial coordination of care. Provides recommendations for integration of clinical social work practice at site. Ensures necessary resources for patients and families are identified in the community, and builds necessary relationships within the community to assist in transitions of care.
Job Responsibility

Leads the Social Work Services group by developing, communicating, and building consensus for goals/programs that support business or region.

Develops and articulates a short-term (6 months – 1 year) strategic vision for areas of responsibility.

Develops and monitors strategic operating goals, objectives, and budget; reports operational performance, justification and/or corrective action.

Oversees development, implementation, administration, and evaluation of Social Work services and programs.

Develops and oversees financial management including budget, salary, capital, and, revenue projections, when applicable.

Oversees daily operations to optimize operational service; monitors operations, evaluates performance and implements improvement plans.

Accountable for the development of processes and workflows that assure safe and timely discharge planning.

Establishes policies and procedures for the efficient, effective and professional operation of the department; selects, develops, manages, and evaluates direct-reports.

Ensures compliance with applicable government laws and regulations, and accreditation standards; works collaboratively with site Quality Management and other departmental leaders and Directors on quality improvement initiatives.

Monitors professional and clinical practice and provides applicable education and training to social workers and facility personnel; ensures professional credentialing of all licensed staff on an ongoing basis.

Ensures timely completion of competency assessment/performance appraisals according to institutional policies and procedures.

Job Qualification

Master of Social Work required.

Current License to practice as a Clinical Social Worker in New York State required, plus specialized certifications as needed.

8-12 years of relevant experience and 7+ years of leadership / management experience, required.

The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.When determining a team member’s base salary and/or rate, several factors may be considered as applicable (e.g., location, specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget and internal equity).
The salary range for this position is $91,400-$158,100/year
