Admissions Coordinator – Elderly Care Facility

A LONG STANDING HOSPICE AND CARE FACILITY WITHIN THE WOODS CROSS AREA IS LOOKING TO BRING ON AN ADMISSIONS COORDINATOR TO JOIN THEIR TEAM!

Schedule:

• Rotating Weekly Scheduled – Mon-Fri either… 8:30-5 – 9:30-6 & 10-6:30
• 1 Scheduled weekend every 4-5 Weeks

This is an onsite role in Woods Cross, UT

Pay: $19/hr

4-Month Contract to Hire

Description:

• Coordinates admissions for homecare, hospice, and skilled nursing facilities
• Obtains pertinent pre-admission documentation from referral sources to determine if agency can care for the patient adequately
• Review clinical documentation to determine level of care needed
• Verifies physician orders
• Develops and maintains relationships internally and with community partners
• Work closely with referring hospitals, physicians, skilled nursing facilities, patients, and families

Additional Skills & Qualifications:

• High school graduate or equivalent
• CNA/MA preferred, but not required
• Demonstrate excellent customer service both in-person and over the phone.
• Ability to work independently and as a part of a team.
• Basic computer knowledge
• Demonstrate excellent organizational and analytical skills.
• Ability to multitask in a fast-paced environment
• Good verbal and written communication skills
• Minimum 1 year experience in healthcare
• Knowledge of Skilled Nursing, Homecare, and Hospice criteria

Experience Level:

Entry Level – Intermediate
