Preceptor Home Health & Hospice is growing!

We are a home health, hospice, and palliative care agency providing services in patient homes in 15 counties throughout Wisconsin. Patient homes include private residences, assisted living facilities, memory care units, and any other place they may call home. Preceptor strives to provide high quality care through our multifaceted programs. We have branch offices located in Germantown, Fond du Lac, Waukesha, Sheboygan, and Appleton.

To best provide 24/7 care for our clients, we are looking to add to our After-Hours team.

We are looking for a dedicated and independent Registered Nurse to cover our Northern Territories!

Shifts are Monday through Thursday from 4 PM to 7:30 AM and from 4 PM Friday to 7:30 AM Monday on alternate weeks.
During this time, you will have both home health and hospice visits. Some of the visits will be scheduled visits and some will be on-call.

SIGN-ON BONUS OF $5,000-$7,500 OFFERED

Job Requirements:

• Carry a current RN licensure in the State of Wisconsin

• 2 years of RN level nursing experience preferred

• Excellent communication skills with patients, families, facility staff, doctors, and teammates

• Ability to work independently

• Good organization and effective time management skills

• Willingness to travel to meet patient needs

• Compliance with documentation standards and regulations

• Prior home health experience preferred, but does not affect eligibility to apply
