JOB RESPONSIBILITIES:
Provide education about asthma and self-management to patients with asthma, their families, and/or caregivers and support systems.
Provide education about the asthma disease process, exercise, medications, complications and their prevention, goal setting, psychosocial adjustment, and other related topics.
Conduct home visits to reinforce family-patient asthma management skills including:
Conducting in-home environmental assessments to identify potential asthma triggers.
Assisting patients and their family to identify their personal asthma trigger(s)
Understand, identify and educate about indoor and outdoor allergens and/or irritants that can make asthma worse and ways to reduce exposure to these triggers.
Process referrals of SSCHC to receive spirometry, administer spirometry, and document appropriate findings in the EHR.
Assist Chronic Care Department staff as needed for coverage (glucometer teachings, Blood pressure cuff teaching, HTN prevention and DASH diet education, facilitating appointments with Chronic Care team).
Implement asthma education plan for patients.
Document appropriately in the medical record in a timely, accurate manner.
Evaluate the outcome of the education based on outcome criteria and adjust the educational plan as necessary.
Work closely with other members of the patient care team, including physicians, other providers, nursing staff and other essential staff members.
Develop and conduct group teaching sessions.
Develop and conduct education sessions for the SSCHC community as needed.
Participate in and provide expertise in clinic projects as appropriate.
Seek and participate in continuing education opportunities related to Asthma care as able.
QUALIFICATIONS:
High School Diploma or GED.
experience in community-based agency or community health organization preferred.
Ability to present information to individuals and groups.
Experience in the clinical setting with a focus on asthma, high blood pressure patient education, internal medicine, nutrition, adult and child health education, chronic disease management or public health preferred.
Ability to interact with patients and families of various cultural backgrounds.
Bilingual (Spanish/English) communication skills required, verbal and written.
<st1:place new=”” roman=”” style=”font-family: ” times=””>Valid Wisconsin</st1:place> driver’s license, use of an automobile and current automobile insurance.
Excellent time management skills and the ability to multi-task.
Basic computer skills.
Maintain BLS Certification.
