Care Support Coordinator

Location: Must live within 50 miles of a Pulse Point.

The Care Support Coordinator is responsible for proactive and/or retroactive patient scheduling. 

Sign-on Bonus: $500.00

How you will make an impact: 

  • Scheduling patients so that home-based provider availability is maximized and to ensure that patients are seen at the right time.
  • Participate in training sessions and review job aides.
  • Provide support to providers by answering emails and inbound call transfers.
  • Support the transition of care process.
  • Escalate patient needs to the appropriate members of the care team.
  • Accurately and concisely document patient feedback and special needs during each call in our computer systems, enabling our market-based clinicians to provide excellent care during each appointment.

Minimum Requirements:

  • Requires a High School diploma or GED and up to 1 year of experience in an automated customer service environment; or any combination of education and experience which would provide an equivalent background.

Preferred Skills, Capabilities and Experiences: 

  • Bachelors degree preferred.
  • Working knowledge of the healthcare industry is strongly preferred.
  • Exceptional oral and written and communication skills preferred.
  • Working knowledge of the healthcare industry preferred.
  • Previous experience and success in a telephonic customer service call center preferred.
  • Durable data entry skills and ability to navigate multiple computer screens preferred.
  • Epic EMR experience preferred.
