Schedule: 1st shift, Monday-Friday, plus on call rotation

Mileage Reimbursement is provided!

Earn $19.05/hour.

Position Details

The Case Manager plays a pivotal role as the recovery coordinator for our community-based Dual Diagnosis Treatment Team (DDTT) serving the north-central and northwestern regions of Pennsylvania. The DDTT focuses on a recovery-oriented approach supporting individuals diagnosed with both a mental illness and intellectual disability.

Duties and Responsibilities

• Collaborate with other members of the team to implement recovery-oriented, person-centered treatment and support plans.
• Performing initial intakes, assessments, and timeline summaries.
• Provide intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, and natural support residences.
• Provide individual and group treatment and support according to each individual’s/family’s needs.
• Support data collection by monitoring interventions and providing feedback to the treatment team regarding client progress.
