What is the Hoarding Intervention and Treatment Program?
Catholic Charities’ Hoarding Intervention and Treatment Program is the only program of its kind in the nation, offering a comprehensive, holistic approach to addressing hoarding disorder. This innovative program provides long-term support by combining case management, in-home coaching, therapy, and community collaboration to improve clients’ lives. We empower individuals to regain control of their living spaces and enhance their quality of life.

What Will I Do as a Hoarding Case Manager?
In this role, you will:

  • Collaborate with Home Coaches and Therapists to develop individualized care plans for clients.
  • Conduct client inquiries and in-home assessments to understand clients’ physical, social, and financial needs.
  • Assist clients in accessing essential services, such as Medicare, food assistance, family care, and disability programs.
  • Monitor client progress by evaluating treatment outcomes, advocating for additional resources, and providing personal support during crises.
  • Maintain accurate records by reviewing case notes, logging events, and documenting reports.
  • Ensure quality service delivery by partnering with program staff and external service providers.

Qualifications:

  • Bachelor’s degree in Social Work, Psychology, Sociology, or a related field.
  • Prior experience working with older adults, individuals with disabilities, or people with cognitive, physical, or mental health challenges.
  • Valid driver’s license, reliable transportation, and vehicle insurance (liability coverage: $100,000/person and $300,000/accident).

Salary Range:

  • $41,000
