Under the general direction of the Director of Home Care/Long Term Care, this position is responsible for the management of an assigned patient care service area, comprised of nurses, hospice aides, social workers, chaplains, volunteers, and administrative staff. Directly supervises the assigned RN Case Managers, Hospices Care Aides and administrative staff while providing operational oversight to the assigned teams’ social workers, chaplains, and volunteers. Responsibilities include but are not limited to, utilization review of costs and services ordered, hiring, supervision and evaluation of staff and coordination of team activities to ensure quality, cost effective patient care. Make recommendations and assist in formulation of team objectives, policies, and plans.

PRIMARY DUTIES AND RESPONSIBILITIES:

  • Coordinates, plans, organizes, implements, and evaluates quality patient care with the interdisciplinary team. The Manager will track key quality indicators and direct performance improvement initiatives in response to complaints and negative feedback from patients, caregivers, and customers.
  • Maintains quality staff by supervision, orientation, training, evaluation, and support of the interdisciplinary group.
  • Collaborates with members of the Interdisciplinary Group (IDG) to resolve or escalate problems related to patient care and customer satisfaction. Responsible for overseeing service recovery relative to patients, caregivers, and customers.
  • Resolves problems to ensure continuity of quality patient care and optimal team function.
  • Collaborates with Psychosocial Services, Spiritual Services and Volunteer Program Manager and Bereavement Services for professional guidance in resolving patient care and/or caregiver issues.
  • Monitors clinical treatment and progress of specialized patient population; collect and evaluate clinical demographic data; audit patient charts, inform Medical Director or designee of patient status/needs. In collaboration with interdisciplinary group, request physical assessment of patient status as necessary.
  • Makes patient assignments based on daily acuity and/or geographic location and apprise Director of Home Care of staffing patterns, needs.
  • Facilitates weekly IDG meetings.
  • Participates in public speaking, community relations and seminars, as appropriate.
  • Develops areas of expertise in Hospice skills and educates staff and community in these areas.
  • Meets on a regularly scheduled basis for planning, supervision, and consultation with the Director of Home Care.
  • Responsible for maintaining the quality of the physical workspace.
  • Maintains quality staff by supervision, orientation, training, evaluation, and support of the nursing and administrative staff assigned.
  • Gives appropriate updated reports to the Director of Home Care regarding risk management issues, patient care, customer relations and budgetary issues.
  • Serves as a resource person in the nursing specialty area and the organization for patients, caregivers, nurses, other health team members and community agencies. Advise patients/caregivers regarding appropriate community agencies and health care support programs.
  • Informs Director of Home Care on staffing patterns and or patient/customer needs.
  • Attends the weekly IDG team meetings to provide performance feedback, address budgetary issues and to serve as a resource to the IDG members.
  • Prepares performance appraisals and provides performance coaching and/or administers disciplinary action for the nursing and administrative staff.
  • Reviews and approves requests for paid time off and the scheduling of nursing staff to ensure staff coverage meets current patient needs and census.
  • Manages budget to include labor costs and other controllable expenses.
  • Responsible for ensuring accurate and timely completion and submission of assigned employee’s payroll information.
  • Ensures organization standards are consistently maintained and that corrective action is taken as needed.
  • Demonstrates knowledge and ensures compliance with all applicable state and federal regulations and guidelines and for patient care and hospice program management.
  • Ensures that patient occurrences, compliments, and complaints are addressed and documented per organization standards.
  • Provides weekend and/or holiday coverage as directed.
  • Performs other duties as assigned.

QUALIFICATIONS:

REQUIRED LICENSES/CERTIFICATIONS:

  • Current licensure as registered nurse in State of Florida.
  • Current State of Florida driver’s license with driving record acceptable to insurance carrier.

EDUCATION/EXPERIENCE:

Any combination of education and/or experience that would provide the required skill and knowledge for successful performance would be acceptable.

Typical qualifications would be equivalent to:

  • Possession of a degree from an accredited school of nursing in a 2 (two) year program or a Bachelor’s degree in nursing.
  • Three (3) years of clinical nursing experience, with at least two (2) years hospice nursing care preferred.
  • At least one (1) year of supervisory experience required.
  • Certification as a Hospice and Palliative Care nurse (CHPN) preferred.

KNOWLEDGE/SKILLS/ABILITIES:

  • Principles and practices of nursing, and hospice care.
  • Supervision of staff.
  • Working long irregular hours.
  • Effective written and oral communication skills.

Participates in organizational operations so that:

  • Operational systems are maintained in regard to Federal, State hospice standards.
  • Compliance with Community Hospice Pharmacy and State regulations in regard to obtaining, disposing, dispensing pharmaceuticals.
  • Takes initiative in addressing areas of need noted regarding facility operations/services, advising appropriate department for resolution.

Participates in total organization program so that:

  • Demonstrates professional relationship with internal and external customers.
  • Promotes organization image within Community Hospice and the community.
  • Uses chain of command for problem solving.
  • Demonstrates appropriate dress and grooming.
  • Displays courteous, tact, patience, respect.
  • Maintains a good attendance record; notifies organization of absences according to policy.
  • Participates in organization Quality Assurance as appropriate.
  • Follows and supports the Clinical Care standards as established.

MEDICAL CLEARANCE:

  • Must have medical clearance for communicable diseases before direct patient contact.
  • Current immunizations including but not limited to TB, Hepatitis, and Flu.
