    The Role

    As a Clinical Pharmacist supportingMedicine and Cardiology services during evening shifts, you will be a key clinical resource for providers, nurses, and patients across the inpatient setting. Your work directly supports safe,evidence basedmedicationuseand continuity of care during a pivotal time of day.

    In this role, you will:

    • Providecomprehensive clinical pharmacy servicesto adult medicine and cardiology patients in an acute care academic setting

    • Participate in interdisciplinary collaboration with physicians, advanced practice providers, nursing, and care coordination teams

    • Optimize pharmacotherapy forcomplex cardiovascular and medical conditions, includinghigh riskmedication regimens

    • Review and verify medication orders with a strong focus on safety, efficacy, andguideline directedtherapy

    • Serve as aclinical consultantduring evening hours, supporting therapeuticdecision makingand urgent clinical needs

    • Contribute to transitions of care, medication reconciliation, andpatient specificeducation as appropriate

MINIMUM REQUIREMENTS

Education: Bachelor’s in Pharmacy or Pharm. D Degree required.

experience: PGY-1 residency required or equivalent hospital experience. (3.years). PGY-2 residency is preferred for specialty areas.

Licensure: Commonwealth of Virginia State Board of Pharmacy. License required. American Heart Association (AHA) Health Care Provider. BLS certification required

PHYSICAL DEMANDS

Job requires sitting for prolonged periods; Repetitive motion: typing. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull < 20lbs. May be exposed to fumes, chemicals, blood/body fluids and infectious disease.</p>

The starting base rate for this role is $64.37 hourly. Individual compensation will be determined by the selected candidate’s qualifications, previous work experience, and/or education.
