Clinical Pharmacist: Signature Healthcare (On-site)

position SUMMARY:

Clearway Health is seeking a Clinical Pharmacist!This position ensuresoptimalpatient outcomes related to chronic and specialty drug-related disease states. Your tasks include preventing or resolving medication-related issues andoptimizingdrug therapy. You serve as a resource and drug expert to patients, families, physicians, nurses, and other healthcare professionals. Additionally,playsan integral role in educating pharmacists, residents, and students. As needed,mayalso be involved in coordinating the acquisition and distribution of drug therapy.

This position will be located on site at our client partner, Signature Healthcare, founded as Brockton Hospital in 1896, a not-for-profit, integrated healthcare system providing a wide range of services to patients of all ages throughout Southeastern Massachusetts. ESSENTIAL RESPONSIBILITIES/DUTIES:

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

EXPERIENCE:

• 2+ years of work experience in ambulatory care pharmacy setting post PGY1 training or 3+ years post PharmD is preferred.

KNOWLEDGE AND SKILLS:

• Demonstrated expertise in medications, drug interactions, medical procedures, therapies, diagnoses, and disease states is essential.

• Exceptional interpersonal skills for effective communication with diverse stakeholders including hospital staff, patients, families, physicians, and colleagues.

• Ability to effectively manage multiple tasks, prioritize duties, and adhere to deadlines.

• Organized work approach with a proficiency in maintaining accurate documentation.

• Strong verbal and written communication skills; proficiency in additional languages is advantageous.

• Self-directed with strong self-management abilities; capable of identifying and addressing issues independently.

• Proven track record in process improvement and a keen eye for detail and precision.

• Proficiency in Microsoft Office programs (e.g., Word, Excel, Outlook) and web browsers.

Special Working Conditions (On-call, travel, shift, coverage):

• Clearway Health Specialty Pharmacies, serving patients in hospitals, health systems, and communities, may require employees at client sites to work on company-recognized holidays.Up to 10%travelmay berequired.
