We are looking for enthusiastic, flexible and highly motivated Clinical Support Workers to join our friendly and supportive community Out of Hours Nursing team in South lakes.You will demonstrate passion and enthusiasm for holistic patient centred delivery of care within the community, with a focus on personal and professional development. You will work as part of a dynamic team who give equal importance to staff health and wellbeing, in addition to providing high standard care to our community. The successful candidate will have the opportunity to develop a wide range of clinical skills and knowledge relating to the increasing complexity of care delivered within the community, including wound, pressure ulcer and end of life care.Various hours available.Main duties of the jobThis role requires excellent communication skills and it is essential that you have the ability to work on your own initiative and can work independently. The department is committed to support continuous education and professional development to facilitate advancement in skills such as medication administration and catheter maintenance care.You will work as part of the Integrated Care Community (ICC’s) covering a 24 hour period including weekends and bank holidays, 7 days a week. You may also be expected to work across a variety of DN teams in the Community Care Group to maintain service delivery.Community Nurses work closely with all health and social care disciplinaries to prevent hospital admission, facilitate hospital discharge and promote independence through reablement and management of long term conditions.You will need access to your own transport and the ability to travel as required in order to fulfil this role.About usAnyone newly appointed to the trust will be subject to the completion of a satisfactory 6 month probationary period (this includes bank staff). This will give the new staff member an opportunity to become familiar with the role and working environment.We operate from three main hospitals – Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery.
