1. Purpose of the Consultancy:

Training of Female doctors and other healthcare workers on NCDs and early detection and referral of common cancers

2. Background

Non-Communicable Diseases (NCDs) are a growing public health concern in Afghanistan, contributing significantly to morbidity and mortality. Among these, breast and cervical cancers are leading causes of cancer-related deaths among women. Early detection and timely referral are essential to improving outcomes. However, there is a critical need to build the capacity of frontline healthcare providers, including Community Health Workers (CHWs), midwives, nurses and doctors, including General Practitioners (GPs), to recognize early signs and symptoms, provide appropriate counseling, and facilitate timely referrals.

To address this gap, WHO is initiating two SIX-day training programs aimed at strengthening the knowledge and skills of healthcare providers in NCD prevention and control, with a special focus on breast and cervical cancer.

4. Work to be performed

The primary objectives of this consultancy are:

Output:

To design and deliver a 6-day training of trainers program for trainers of female midwives and doctors. This will take the form of two separate modules:

NCD Module: Two days refresher ToT on Guidelines for NCDs prevention and Control

Cancer Module: Four days ToT on screening, early detection and referral of breast and cervical cancers as well as the early symptoms for the other most common cancer such as Esophageal, Gastric and Colorectal and lung cancers as the most common cancers reported in Afghanistan.

1. To design and deliver two modules of training of trainer’s program for female nurses and Community Health Worker trainers. This will take the form of four days of training on NCDs and TWO days on cancer, with an emphasis on screening and early detection of breast and cervical cancers as well as Esophageal, Gastric and Colorectal and lung cancers as the most common cancers.

2. To develop culturally appropriate and evidence-based training materials tailored to the Afghan context.

3. To enhance the capacity of healthcare providers in patient education including breast self-examination, raising awareness, clinical evaluation, screening, referral, and follow-up for NCDs and breast and cervical cancer.

5. Deliverables:

The consultant will be responsible for:

• Conducting a rapid assessment of needs to tailor the training content.
• Designing a comprehensive training curriculum covering:

o Overview of NCDs and their burden in Afghanistan.

o Risk factors, prevention, and management of major NCDs.

o Early detection and referral pathways for breast, cervical cancers as well as Esophageal, Stomach and Colorectal and lung cancers as the most common Cancers.

o Communication and counseling skills for community engagement.

• Developing training materials, including presentations, handouts, and job aids.
• Facilitating two 6-day in-person training of Trainers workshops in four modules as above-detailed, in Afghanistan.
• Conducting pre- and post-training assessments to evaluate knowledge improvement.
• Providing a final report with recommendations for future capacity-building initiatives.

The consultant is expected to deliver:

• Training curriculum and agenda.
• Training materials (slides, handouts, job aids).
• Pre- and post-training assessment tools and results.
• Final training report including participant feedback and recommendations.

Timeline and Key Milestones:

Month 1:

• Conduct situation analysis and stakeholder consultations.
• Develop training materials and draft clinical workbooks.
• Finalize training materials and workbooks.

Month 2:

• Deliver first training of trainer’s workshop (midwives and female doctors) in the primary care prevention and management of major NCDs and early detection, referral and follow-up of common cancers in primary care including simulated practical components.

Months 3-4:

• Review lessons learned from the ToT workshop and make any necessary adjustments to the training materials (slides, handouts, job-aids)

Months 5-6

• Deliver second training of trainers workshop (female nurses and CHWs) in prevention and community management of major NCDs and prevention, early detection and referral of common cancers

Months 7-8:

• Review lessons learned and make any necessary final adjustments to training materials (slide deck, handouts, job-aids)
• Produce final report with recommendations

8. Specific requirements

Qualifications required:

• Master’s degree in public health and or medicine.

Experience

Essential

• From 5 to 10 years of experience in NCDs management and treatment, cancer control and/ or health education.
• Demonstrated expertise in designing and delivering training programs for healthcare providers, preferably in fragile or low-resource settings and specific to Afghanistan.
• International Experience is a requirement for international consultancies.

Desirable

• Knowledge of the Afghan health system and cultural context is considered an asset.

Skills / Technical skills and knowledge:

• Strong facilitation, communication, and report-writing skills.

9. Place of assignment

The consultant will perform his duties in a hybrid manner with most of his assignment to be performed remotely from his home country and visit Kabul twice for two weeks to facilitate TOTs physically

.
