We’re looking for an Habilitative Coordinator/Case Manager to direct and coordinate the delivery of habilitative services -all while ensuring we provide a meaningful life in a caring community!
? A Snapshot of Your Day (Essential Job Functions):
Think of yourself as a Life Pathway Architect and Staff Champion!
* Empowerment Enthusiast: You’ll support and encourage individuals of all ages and abilities in making meaningful choices to determine their pathway in life.
* Compliance Coordinator: You’ll coordinate the delivery of habilitative services, making sure we stay compliant with all regulatory requirements (you’re our internal superhero!).
* Behavioral Trainer: Get ready to train our awesome staff in behavior modification and/or intervention techniques.
* Program Pioneer: Manage the development of individual-centered programs and evaluate their effectiveness.
* Records Rockstar: Document individuals’ progress and keep records maintained like a pro.
* On-Call Lifeline: You’ll be on-call to provide assistance and guidance to staff when those tricky problems pop up.
SCHEDULE: generally days, salaried, on call responsibilities to support the departments/agency which can include other hours (evenings, overnights, weekends) as needed
