Equity, Health Inclusion and Wellbeing Leads are experienced practitioners with a working knowledge ofreducing health inequalities and increasing proactive care;Focusing on ensuring services provided by Sirona are able to identify and improve access and delivery to underserved populations and groups and promote a proactive care approach for people on the caseload, working in partnership with local communities using a strengths-based model, such as Asset Based Community Development.Equity, Health Inclusion and Wellbeing Leads support the delivery of the personalised care agenda and the development of community capacity in each locality.Main duties of the job(this list is not complete please see job description for full details)Line Management

  • Line Manage Support Workers and community navigator (Health Link) workers working within the team.
  • Provide supervision and support on-going professional development.

Providing Advice and Support

  • Provide advice in relation to complex professional matters and specifically relating to the impact of inequality in service access and delivery.
  • Carry out a range of specialist duties, including providing professional, data gathering, analysis and reporting, recording, interpreting and recommending further courses of action for Sirona community services.

Reducing Barriers to Access

  • Work across Sirona services to remove barriers relating to access to care and service delivery tosupport people from undeserved communities and groups to access community services,developing solutions taking into consideration protected characteristics.
  • Design and deliver placed based services to improve service access and delivery in identified underserved communities and groups.

Evaluating Impact and Sharing Learning

  • Evaluate the impact of initiatives in terms of anticipated benefits and actively share the learning from initiatives across the organisation. Actively seek feedback from services and communities/groups on the impact and benefit of the Equity and Health Inclusion team.
  • Scope, develop and implement the educational development requirements in relation to proactive care and health inequalities

  • Experience of working within Safeguarding, Data Protection, Continuing Health Care and Mental Capacity Act framework
