Health Information Management Specialist in New Haven, Connecticut

Reporting to the Manager of the Health Information Management Department, the HIM Specialist is responsible for release of Cornell Scott-Hill Health Center (CS-HHC) patient information to duly authorized recipients within the time periods established by the Department. The HIM Specialist will also take direction from the Senior HIM Specialist as it relates to issues of workflow. Duties are performed in accordance with CS-HHC policies and procedures and state and federal law and regulations. The HIM Specialist shall maintain records of information released in accordance with HIPAA requirements, coordinate the activities of other staff members engaged in release of information activities, and prepare summary reports of activities performed for quality improvement purposes.

Responsibilities include but are not limited to:

  • Coordinates with collaborating physicians and/or designee on all home health orders and ensures that the physician’s signature is documented.

  • Processes and links orders into the patient’s medical record. Ensures accurate and timely delivery of orders to proper agencies.

  • Tracks patient orders sent to agencies to ensure orders are processed as ordered and results are received timely.

  • Links patient documents to the appropriate patient charts in the EMR that include, but are not limited to test results, DME orders, and home health orders. Sends documents to providers in the EMR for review, as needed.

  • Returns completed orders to agencies by fax, email, or other electronic method. Cross-train as back up for other positions as assigned.

  • Organizes, attends, and participates in meetings with home health representatives to review processes and ensure timely return of orders.

  • Verifies patient information; collects and assembles patient medical records; obtains discharge records; performs records processing and enters information into proper patient record and information system.

  • Responds to inquiries and requests for information from organizations, patients, families and staff in accordance with legal, regulatory and departmental policies and procedures; researches inaccuracies; adheres to privacy, security and consent mandates in the completion requests.

  • Conducts audits verifies the accuracy of scanned documents and source documents and ensures documents are legible and properly indexed for accurate retrieval; identifies and documents inaccuracies in accordance with established criteria; recognizes trends in scanning errors

  • Advises healthcare staff and supervisors as needed, to wit: Policies and regulations, old and new workflows within the HIM Department, proper scanning of documents, documents that require provider input, advising employees how to properly file an RL6 report concerning patient privacy; resolves home health order issues as it pertains to the regulation, engages management concerning new workflow possibilities.

  • Gathers and generates reports on various department metrics for review by management; recommends processes and procedures to improve departmental performance.

  • Completes special projects as assigned.

  • Performs other duties as required.

    Requirements:

  • High School Diploma/GED required. Some college course work and medical terminology preferred.

  • A minimum of one (1) years of relevant experience.

  • This position requires attention to detail and the ability to work independently with minimal supervision.

  • Excellent computer skills and ability to use office programs are essential.

  • Written and verbal communication skills are required.
