Job description
As a Health Referral Advisor, you will support Brio’s mission to improve community health and wellbeing in alignment with Cheshire West and Chester Council’s ‘Play Your Part Towards a Stronger Future’ Plan. Through our Integrated Wellbeing programme, you will empower individuals to make positive changes, reduce health inequalities, and enhance their quality of life.You will be part of a small team who provide high-quality person-centred care, triage direction, client advice and administration for Brio’s Wellbeing programmes.
