We welcome applications from existing health visitor practitioners, who may be looking for a change of location to students who are currently completing the specialist community public health nursing course.Student SCPHN HV’s due to qualify can also apply.We are passionate about the support and development we give our employees and as a member of the Trust and Children’s Integrated Health and Wellbeing Service we are committed to your development and will create a dedicated personal development plan for you, to make sure you are fully supported in all your career ambitions.The sky is the limit for you here in Bolton!Must already hold the SCPHN qualification to apply.Main duties of the jobAs a Specialist Community Public Health Nurse, you will have excellent communication skills and a broad knowledge of working in the community with children and their wider family.You will be a solution focused team player who will support the delivery of the universal offer in a 0-19 integrated neighbourhood team.You will manage a universal plus and partnership plus caseload, working collaboratively with other agencies to ensure protection and the best health outcomes for children and families in Bolton. As a Specialist Community Public Health Nurse you will work within the community dealing with clients from diverse backgrounds with varying levels of need.Specialist Community Public Health Nurses in Bolton deliver an evidence based service using a wide range of assessment and intervention tools as well as providing excellent support alongside a wider multi-disciplinary 0-19 service.Some of the benefits of working for Bolton Hospital NHS Foundation Trust include:

Universal health and development reviews are key features of the programme and aim to assess children, young people and families strengths, needs and risks and provide parents, cares, children and young people with the opportunity to discuss their concerns and aspirations. Growth and development are assessed at key points with the aim of promoting health and wellbeing and the early detection and management of health needs. Additional support is provided at key times to protect children, young people and families who are experiencing harm or are at risk of suffering significant harm. It supports a model of progressive universalism; a core universal programme for all children with additional contacts for those with identified need.
