This position would cover the Laurel DE area and some of the surrounding areas (Laurel, Millsboro, Rehoboth Beach, Salisbury MD). Primarily covering Worcester County, Wicomico County in MD and Sussex County in DE.

A DAY IN THE LIFE OF A Home Care Registered Nurse/Supervisor

• Makes initial contact with the patients and explains services offered
• Completes start of care assessments and documentation to onboard new patients
• Designs an individualized plan of care for each patient
• Instructs assigned care team members on the needs and care plan details
• Reports progress of care to involved parties
• Follows up and maintains relationships with clients, always advocating on their behalf
• Establishes relationships with community partners and referral sources
• Keeps Clinical Leadership and Office Administrator abreast of patient condition and/or medical changes
• Reviews caregiver documentation
• Takes part in interdisciplinary team meetings to ensure needs of all patients are met
• All other duties as assigned by Administrator and job description

Requirements:

• Must possess an active RN License covering Maryland and Delaware.
• Minimum of 1 year in a Nursing role- LPN to RN acceptable
• Minimum of Associates Degree in Nursing
• Comfortable working collaboratively in cross-functional teams
• Ability to communicate effectively, both verbally and in writing, with all levels of an organization
• Excellent presentation skills
• Adequate transportation for home visits with mileage reimbursement
• Computer/iPad Technology skills for documentation purposes
