We’re seeking an experienced Home Health LVN for a field-based assignment in Petaluma, CA covering Sonoma County. This is an excellent opportunity for LVNs with recent home health experience who are comfortable managing patient visits independently and documenting in HCHB.

Location: Petaluma, CA

Setting: Home Health

Schedule: Monday–Friday, Days

Contract Length: 13 Weeks

Coverage Area: Sonoma County

• May be asked to assist in other territories within the agency service area
• Travel radius may extend up to approximately 60 miles from the facility
• Mileage reimbursed at $0.76/mile
• Productivity expectation: 32 points per week

Pay Options

Travel LVN

• $2,080/week
  • $880 hourly
  • $700 housing allowance
  • $500 meal allowance

Local LVN

• $48/hour

Requirements

• Active California LVN License
• Recent Home Health experience required
• HCHB (Homecare Homebase) experience required within the last 3 years
• Strong clinical assessment and documentation skills
• Ability to manage a field-based caseload independently

Additional Requirements

• Must meet all federal, state, and facility requirements
• Understanding of HIPAA and patient confidentiality standards
• Must be at least 18 years of age
