- Valid Registered Nurse (RN)
- Previous home health experience preferred, but not required
- Previous experience with managing ventilators and enteral feeding devices, but not required
- Experience with clients who show challenging behaviours is preferable
Communication Skills: Strong communication and interpersonal skillsKey Skills: Ability to work independently, empathetic, client-focused approach to care, reliable transportation and willingness to travel to clients’ homes within the designated service areaShifts: Day and Night shifts available.KEY DUTIES AND RESPONSIBILITIES
- Perform assessments of client’s physical and emotional health in the home setting
- Administer treatments as prescribed by MDT
- Provide airway management, enteral feeding and other specialised services
- Monitor and manage chronic conditions and illnesses
- Educate clients and their families about health conditions and treatment plans
- Collaborate with MDT, client and family members to ensure the best possible care outcomes
- Keep accurate, up-to-date client records in accordance with company policies and regulatory standards
- Offer emotional support and reassurance to clients and families during challenging times
- Offer training and competency assessment to healthcare assistants
