PACE Department is seeking a Clinic Registered Nurse to join its team in San Francisco’s Inner Richmond neighborhood. Reporting to the Director of Nursing , the Clinic Registered Nurse offers nursing services, either directly or indirectly, to all participants at the Day Health Center.

This position is scheduled for 8 hours per week, with work performed on Saturdays.

PACE
On Lok pioneered the model of care known as “Program of All-inclusive Care for the Elderly” or PACE. Institute on Aging operates a PACE Center in the Richmond District of San Francisco. PACE allows seniors who are ill or disabled to live in their own home, whether home is a family residence, apartment, retirement village or hotel room.

IOA’s PACE Center is a lively place where participants have access to medical care, social activities, exercise and meals. Participants may attend our PACE Center one to five days a week, based on the team’s recommendations and the senior’s needs and interests. IOA’s staff members are sensitive to the needs and changes of each participant”s health and emotional state. This comes from many decades of experience in senior care.

Schedule: This position is scheduled for 8 hours per week, with work performed on Saturdays.

SPECIFIC RESPONSIBILITIES:

Nursing Care

  • Provides nursing advice and care to manage symptoms for minor illnesses, injuries, and chronic illnesses of participants.
  • Collaborates with PCPs to evaluate participant condition, provide health education, care coordination, home visit follow-up and/or other treatment recommendations as indicated.
  • Ensures implementation of evidence-based clinical guidelines and nursing practices across the PACE care settings.
  • Provides guidance to Home Care Workers with caregiving challenges including after-hours staffing issues when needed.
  • Supports transitions of care by participating in hospital discharge planning to reduce avoidable readmissions and adverse outcomes.

Quality Assurance

  • Ensures nursing care, documentation, and medication management practices comply with PACE regulatory requirements, state licensure standards, and organizational policies.
  • Participates in Quality Improvement (QI) initiatives, audits, and performance projects to improve clinical outcomes and participant satisfaction.
  • Report and investigate clinical incidents, near misses, and safety concerns in accordance with organizational policy.

Other:

  • Shall be immediately accessible by telecommunications during operating hours when patients are receiving services.
  • All other reasonably related jobs as assigned.

BACKGROUND AND EXPERIENCE:

Education

  • Graduate from an accredited school of nursing

Licenses/Certifications

  • Unrestricted license as an RN in the State of California
  • Current BLS certification or willing to receive training within 6 months of hire
  • Public Health Nurse (PHN) certification preferred

Experience

  • Minimum of two years of experience within the last five years in a home health agency, primary care clinic, or health facility.
  • Sufficient background knowledge and expertise in clinical decision-making for the elderly population to meet the needs of the patients and to contribute to Quality Management review and evaluation.
  • Must drive or use public transportation for home visits.
  • Computer literacy and proficiency using electronic health records.
  • Language capability – Spanish, Russian or Chinese preferred.

COMPENSATION:

Range: $56 – 62/hrly
