Preceptor Home Health & Hospice is growing at a rapid pace!
If you are an RN and interested in management for a fantastic hospice team, come join our team!

We are a home health, hospice, and palliative care agency providing services in patient homes in 15 counties throughout Wisconsin. Patient homes include private residences, assisted living facilities, memory care units, and any other place they may call home. Preceptor strives to provide high quality care through our multifaceted programs. We have branch offices located in Germantown, Fond du Lac, Waukesha, Sheboygan, and Appleton.

Responsibilities:

• Receiving all new referrals and reviewing patient information to determine hospice eligibility within 2 hours

• Collaboration with our Medical Doctors

• Ensuring the plan of care is implemented and updated as needed. Coordinating case conferences and chart audits. Maintaining a high level of clinical excellence and compliance.

• Processing orders and workflow

• Triaging calls from facilities, patients, families, caregivers, and doctors, and completing patient visits if the need arises

• Recruitment, orientation, training and retention of staff

• Maintaining a CAPLICO culture/values.

• You will also be added to the Administrator On-Call rotation (about every 8 weeks)

• Community education
  

To be successful in this role, you will need the following:

• Valid RN licensure in the state of Wisconsin???????

•   ???????Hospice experience required

• ????    Two years of nursing experience required

• Sound nursing skills

• Ability to work independently

• Excellent written and oral communication

• Exceptional time management skills

• Great people skills
