Mosaic is looking for a Licensed Practical Nurse (LPN) Float Pool to join our team!

In this role, you will assist people with intellectual and/or developmental disabilities in achieving their best possible health. This may include advocacy with healthcare providers, administering medications and/or injections, observing and monitoring each persons health status and communication with support team on any changes.

What YOU’LL will do:

• Provide nursing support and consultation with the state’s Nurse Practice Act and physician orders.
• Ensure individuals receive routine and preventative health services and medication administration is completed accurately according to regulations.
• Provide health related education to those served and those who serve in the agency.
• Maintain and review electronic healthcare records to monitor pertinent health status, provide accurate and through documentation and update agency leaders as needed
• Build effective relationships with colleagues, persons served and their providers and families/guardians.

SCHEDULE: DAYS, 8 hour shifts, can work up to 24 hours a week – required to work at least 24 hours a quarter, NO HOLIDAY or WEEKEND requirements, NO ON CALL, travel between Grand Island and Axtell may be required with an agency vehicle.

What YOU’LL Need:

• State license required in nursing in good standing, ASN required
• A minimum of 1 year general nursing or 3 years CNA/CMA experience is required.
• Must be able to lift, push and pull at least 50 pounds.
• Valid Driver’s license
