Job Description

Activa Home Health, a licensed and Medicare-certified home health agency, is seeking experienced Field Licensed Practical Nurses (LPNs) to provide high-quality home health care throughout Palm Beach County.

Responsibilities

• Provide skilled nursing care to patients in their homes per the physician’s plan of care

• Perform nursing visits, treatments, and documentation in compliance with Medicare and agency standards

• Monitor patient condition and report changes to the supervising RN or physician

• Educate patients and families on care, medications, and treatment plans

• Maintain accurate and timely clinical documentation

Qualifications

• Active Florida LPN license (required)

• Home health experience preferred

• Strong clinical, communication, and documentation skills

• Ability to work independently in the field

• Reliable transportation and valid driver’s license

• Ability to pass background screening and drug test
