POSITION DESCRIPTION: 

Provides support services and interventions face-to-face, within the home and community for individuals (children & adults) experiencing mental health or substance use-related crisis. This individual will need to be prepared to provide immediate, short-term and evidence-based practices to screen, assess, stabilize, and make appropriate referrals, including crisis stabilization units, inpatient hospitalization, and acute detoxification services. You will collaborate with the Community Support Services and other departments at PSS to support transitions of care for consumers. The Mobile Crisis Team operates 24/7 during the week- days, evenings, and night shifts, and weekend shifts available.

RESPONSIBILITIES:

  • Demonstrate knowledge of:
  • signs and symptoms of substance use disorder
  • therapeutic interventions for individuals diagnosed with serious mental illness including de-escalation techniques
  • signs and symptoms of mental health disorder
  • crisis intervention services on-site and off-site, face-to-face, within the home and community
  • assessing the client’s strengths and mental health needs accurately and objectively
  • providing selected therapeutic communication to clients
  • clients rights and confidentiality
  • typing and basic computer usage
  • willingness to work nights, weekends, and holidays according to on-call rotation
  • the requirement of adhering to policies and procedures of the agency and professional standards
  • Demonstrate skills in:
  • establishing trust between clients and treatment team members
  • providing information and referrals to community services both in person and via the phone
  • assisting in implementation of goals and objectives identified in treatment plans
  • interacting with clients as peers by sharing common experiences
  • assisting clients in understanding and managing their disorder and symptoms
  • connecting with self-help groups when appropriate
  • gaining access to local programs and services
  • clarifying understanding of rights and responsibilities as a recipient of substance use disorder services
  • maintaining strict confidentiality
  • communication styles that generate a positive work environment
  • locating, coordinating, and accessing appropriate services for client
  • Essential functions include the following. Other duties may be assigned:
  • Completion of required assessment screenings, including but not limted to the Columbia Suicide Rating Scale (CSSRS)
  • Engage individuals in the crisis planning process and completion of a safety plan
  • Build rapport to support increased engagement, instillation of hope, and care coordination
  • Follow-up with individuals in-person, via phone, or telehealth within two days of the initial crisis intervention
  • Provide opportunity for warm hand-offs to identified resource needs
  • Provide referral to medication management to monitor medication compliance, education and training about medications, and assessments
  • Referrals to Case Management when appropriate
  • Participate in team staffing to provide peer insight
  • Collaborates with treatment team to increase patient engagement. Attends huddles, weekly team meetings, staffing, clinical supervision
  • OBHP (Other Behavioral Health Professional) certification will be provided by PSS upon hire and is required within three weeks of hire date

QUALIFICATIONS:

  • Bachelor’s degree in social work, psychology, or related human services is required
  • Previous related experience preferred
  • Valid driver’s license, personal owned auto insurance
