What you will doThis position is responsible for regional planning, implementing, and monitoring strategies for quality components of the clinical care delivery in Horizon Primary Health Care program, with the goal of enhanced standardization of clinical processes across Horizon Primary Health Care sites.The Primary Health Care Standardization Coordinator is required to work in collaboration with all areas of the organization, our patients, and their families, as well as with external stakeholders in the development of strategies and their operational implementation. This includes physicians, nurse practitioners, as well as all members of the primary health multidisciplinary team.This role will support and make recommendations to the Primary Health Care Leadership team, regarding clinical quality initiatives with a focus on patient care, medical directives, policies, accreditation, and other performance standards. They will communicate with employees, patients, their family, other healthcare providers and stakeholders as required to accomplish his/her functions.
