Description
- Organizes and conducts Occupational Therapy program to facilitate the rehabilitation of people with physical and/or psychosocial disabilities.
- The Occupational Therapist directs the patient’s participation in selected activities/treatments to restore, re-enforce and correct pathology to promote and/or maintain good health.
- Performs evaluation(s) and develops an effective treatment plan appropriate to a patient’s rehabilitation needs within the care continuum.
- Performs effective treatment techniques in a competent manner.
- Educates patient/family/caregiver in applicable health information, treatment and prevention.
- Initiates and participates in the discharge planning process.
- Performs other department activities such as, but not limited to documentation, billing, support of department operations, etc.
- Performs other duties as assigned.
- Complies with all policies and standards.
- For specific duties and responsibilities, refer to documentation provided by the department during orientation.
- Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients.
- Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
- 100% travel to the patietn’s homes for care.
- Part time 24 hours per week.
- This position is in Home Care Services.
- The territory is for the East Market: Mentor, Ashtabula, Geneva, MIddlefield.
- Patient population will be pediatrics.
