Onsite RN Clinical Care Coordinator

We are seeking a patient-focused clinical professional to coordinate nutrition therapy for pediatric patients with complex medical needs. This role supports care plans involving enteral feeding and specialized medical nutrition, working closely with clinicians, dietitians, and DME suppliers to ensure continuity of care. By monitoring adherence, supply continuity, and clinical changes, the representative supports families through transitions of care and growth-related changes while delivering coordinated, high-quality support.

Must be able to work onsite: 105 Live Oaks Gardens, Casselberry, FL 32707**

What You’ll Do:

* Coordinate nutrition therapy for pediatric patients with complex needs
* Support care plans involving enteral feeding and medical nutrition
* Partner with clinicians, dietitians, and DME suppliers
* Monitor adherence, supply continuity, and clinical changes
* Support families through transitions of care and growth-related changes

What You’ll Bring:

• High school diploma or GED required.
• Must be at least 18 years of age.
• 1+ year of customer service experience
• Typing speed of 25 WPM with 90% accuracy required.
• Proficiency in MS Office: Word, Outlook & Excel
• Effective written and verbal communication skills.
• Ability to obtain, organize and present information
• Ability to handle multiple tasks as assigned.
• Licensed RN, dietitian, or clinical professional
• Experience working with children, nutrition support, or patients with complex medical needs
• Strong care coordination and family engagement skills
