position SUMMARY:
Provide proactive, preventative, and therapeutic nutrition care in a primary care setting, working alongside physicians, nurses, MAs and other healthcare professionals to support patients in managing chronic conditions and promoting overall wellness.
QUALIFICATIONS:
Required:
Registered Dietitian (RD) with current Oregon licensure. Must demonstrate a positive attitude, flexibility, and strong problem-solving skills. Self-starter able to work independently and manage patients in diverse outpatient settings. Requires ability to communicate effectively and concisely with a multidisciplinary team to support coordinated, patient-centered care.
TYPICAL PHYSICAL/MENTAL DEMANDS:
Physical: Light physical demand level. Able to stand, walk, and sit frequently. Possesses adequate reaction time to respond quickly to unexpected patient care situations. Must be able to see, hear and speak with or without assistive devices.
Mental: The ability to instruct others. Socially perceptive. Effective use of computers & programs/software as appropriate to job. Ability to prioritize workload to meet changing needs of patient, staff and community. Must possess a comfort level with public speaking as warranted for education needs of patient, staff and community.
ESSENTIAL JOB FUNCTIONS:
- Provide individualized nutrition assessments, counseling, and medical nutrition therapy (MNT) to patients across the lifespan in primary care clinics.
- Develop and implement evidence-based nutrition care plans for conditions such as diabetes, hypertension, obesity, cardiovascular disease, gastrointestinal disorders, and more.
- Collaborate with primary care providers and interdisciplinary teams to support coordinated, whole-person care.
- Document all patient interactions and interventions in the electronic medical record (EMR) in accordance with clinic policies and professional standards.
- Participate in case reviews, care team huddles, and health promotion initiatives.
- Deliver nutrition education sessions individually or in group settings, both in-person and via telehealth.
- Contribute to the development of nutrition-related programs and resources tailored to the clinic population.
- Maintain up-to-date knowledge of current nutrition guidelines and evidence-based practices.
