PRN Palliative Nurse Practitioner

Washtenaw County

Schedule: PRN; must commit to a minimum of 16 hours per month

On an Average day, you will:

• Align with Vision and Policies: Ensure all activities align with the vision of Elara Caring’s leadership and promote the organization’s philosophy, mission, and policies to maintain quality care.
• Patient and Staff Confidentiality: Protect privacy and confidentiality as per HIPAA regulations.
• Client Assessment and Diagnosis: Conduct interviews, health histories, physical exams, and diagnostic studies to assess physical and psychosocial status, identifying abnormalities.
• Collaborative Care: Work with palliative care and attending physicians to develop and implement treatment plans and coordinate care goals.
• Treatment Implementation: Administer treatments, collect specimens, prescribe medications, and recommend ongoing treatment plans.
• Care Evaluation: Monitor and evaluate the quality of care, suggesting improvements and ensuring alignment with patient needs.
• End-of-Life Care: Assist patients and families with end-of-life discussions, goal setting, and care planning.
• Community Coordination: Plan care using community resources and act as a liaison for continuity of care among healthcare providers and agencies.
• Education and Training: Support in-service training and educate care team members.
• Documentation: Maintain accurate and timely records of all activities.
• Additional Duties: Perform other tasks or projects as assigned.

This role combines patient care, collaboration, administrative responsibilities, and educational support to ensure comprehensive and high-quality care.

What You’ll Need

• Education & Licensure:
• Master’s Degree in Nursing with national Nurse Practitioner certification.
• Active Nurse Practitioner licensure in your state of practice.
• DEA License (required).
• Board certification in Palliative Care (preferred).
• Experience:
• 3+ years of clinical nursing experience, with at least 1 year in hospice or palliative care (preferred).
• Background in Family Medicine, Geriatrics, or Psychiatry is a plus.
• Skills: Commitment to compassionate care and reliable transportation for patient visits.
