<div class="isg-job-description“>The Navigation Outreach Specialist provides individualized assistance to community members to overcome barriers to accessing and completing cancer screening and access to supportive care services. The Navigation Outreach Specialist focuses on connecting with community members due for cancer screening and connecting community members to appropriate resources for cancer screening, as well as, connecting cancer patients to supportive care services to improve quality of life and adherence to cancer treatment. The Navigation Outreach Specialist serves as a critical link between community members, UVA Health, and other community health care systems, organizations and resources.
This is a net new position that is funded as a pilot by directed philanthropic donation. The purpose of this role to evaluate the efficacy of a non-clinical patient navigator who can support the non-clinical pathway of both cancer screening and cancer patients and caregivers. This role will partner with Nurse Navigators, Supportive Care Services, and the cancer screening team. Areas of focus will include supporting initial visit and screening logistics, entre into supportive care (financial assistance, spiritual care, social work, support groups, educational activities, and patient events). This role will also coordinate regular check-ins in partnership with the nurse navigator and follow up on the multiple assessments/surveys required for accreditation and quality care.

  • Collaborate with members of healthcare teams to design and coordinate an evidence-based plan of care. Serve as primary resource for patient to connect them to appropriate resources
  • Work with the community, outside agencies and organizations to promote cancer awareness and education.
  • Help identify barriers that may be preventing patients and families from receiving timely, high-quality cancer screening and supportive care services and identify potential solutions to those barriers.
  • Assists the patient in addressing barriers to care (transportation, language, finances, understanding, etc.)
  • Provide patient education by assisting patients in understanding their care and available resources
  • Assist patients in informed decisions related to cancer screening and supportive care services.
  • Ensure that referrals, procedures, communication, and follow-up occur in a timely fashion through data and problem-solving.
  • Provide extensive understanding of patient care and health care pathways to serve as a conduit between the patient and the health care system.

Position Compensation Range: $45,656.00 – $70,761.60 Annual

MINIMUM REQUIREMENTS
Education: Bachelor’s degree required.
experience: Two (2) years of experience required with a Bachelor’s degree. In lieu of a degree, six (6) years of relevant experience with a high school diploma/GED is required.
Licensure: None required.
PHYSICAL DEMANDS
This is primarily a sedentary job involving extensive use of desktop computers. The job does occasionally require traveling some distance to attend meetings, and programs.
