Field LPN – Home Health Visits (Sebastian/Fellsmere)

position: Per -Diem Field LPN Home Health Visits- Vero Beach
Location: Indian River County

About the Role

As a Field LPN with Activa Home Health, you’ll deliver high-quality, compassionate care to patients in their homes, ensuring their safety, comfort, and satisfaction. You’ll follow individualized Plans of Care under the supervision of an RN to help patients maintain their health and independence.

experience with WellSky/Kinnser and home health documentation is a plus!

Key Responsibilities

• Provide safe, effective nursing care based on individualized Plans of Care.

• Communicate promptly with the clinical team regarding changes in patient condition or emergencies.

• Perform skilled nursing tasks, which may include:

  • Medication administration

  • Wound care

  • Infusion therapy

  • Catheter care

  • Patient transferring and positioning

• Collaborate with the RN Supervisor and interdisciplinary team to ensure coordinated, high-quality care.

• Demonstrate empathy, professionalism, and attention to detail in every interaction.

Requirements & Qualifications

• Active LPN license in the state of Florida

• Wound care skills required

• IV certification preferred

• Minimum 1 year of home health experience preferred

• Excellent communication, teamwork, and customer service skills

• Proficiency with electronic health records (EHR), preferably WellSky/Kinnser
