Assesses spiritual needs of patients and provide appropriate interventions with consideration to growth development needs and age group Receive referrals from all members of the healthcare team, and follow up on these referrals with 24 hours Develops a pastoral care plan for patients by identifying objectives, interventions and desired outcomes Participates in patient care rounds and discharge planning process Communicates appropriate information to the interdisciplinary team through appropriate charting in the patient medical record Educates staff on the role and function of the chaplain Facilitates community relationships for the Hospital and maintains open communication with area clergy. Assists in the education of Clinical Pastoral Education residents and interns as indicated Demonstrates knowledge of the levels of growth, development, and skills for pediatric, adolescent, adult, and geriatric Available for on-call duty (nights, weekends, holidays) as scheduled by the Director/Associate Director of Pastoral Care Demonstrates, Caring behaviors to patients, families and colleagues. Demonstrates respect for human dignity and self-worth and provides spiritual care in a non-judgmental and non-discriminatory manner. Effectively participates in conflict resolution and problem solving Identifies opportunities to provide spiritual care support to enhance the patient experience and exceed their expectations in a timely manner. Operates Spiritual Care Department in a financially responsible manner to ensure any budgetary guidelines are met. Administers Sacraments to patients upon request and makes arrangements to support patients and families of other faiths who request it. Develops and maintains a comprehensive volunteer minister program that supports the Spiritual Care needs, programs, and community outreach. Provides for on-call personnel to ensure spiritual care services are provided during unplanned events. Serves on appropriate hospital committees. Demonstrates commitment to professional and personal growth. Qualifications **What You Will Need:** + Personal commitment to God.Personal integrity, emotional stability, and an ability to work under stress.Effectively relates to persons of diverse religious, educational, and cultural backgrounds.Demonstrates ability to earn respect of nurses, physicians, and other healthcare professionals. + Master of Divinity degree + Minimum of two units of Clinical Pastoral Education + Four units of Clinical Pastoral Education preferred **Job Summary:** Clinically trained minister responsible for providing spiritual care to our primary customers, partners, and team members. Participates as a member of the healthcare team in evaluating spiritual needs of patients, and provides appropriate interventions to meet such needs. Chaplain fosters a sense of mission that enhances a culture of Trust, an experience of Belonging, and a future of Hope within the hospital and healthcare team.
