Job Summary:

• We’re looking for a compassionate professional with a background in social work, counseling, or public health to join our team.
• In this role, you’ll provide direct support, counseling, and case management for individuals affected by STDs and HIV while working closely with healthcare providers and community partners.

Role & Responsibilities:

• Provide case management and counseling for individuals diagnosed with STDs/HIV.
• Connect patients to testing, treatment, and supportive resources.
• Conduct field investigations and outreach to notify individuals who may have been exposed.
• Partner with healthcare providers to ensure quality care and follow-up.
• Offer education and prevention counseling to help reduce reinfection.

Required/Preferred Skills:

• experience in case management or social services.
• Strong communication skills, empathy, and ability to maintain confidentiality.
• Flexibility to travel statewide if needed.

Required Qualifications:

• Bachelor’s degree (preferred in social work, public health or similar)
