Join a well-established and supportive medical team dedicated to providing high-quality care for individuals who have chosen Mosaic. This role is vital to the medical success of those we serve, offering a unique blend of hands-on clinical care and medical oversight. You will act as a medical guide during crises and a consistent advocate for individuals during critical care appointments.
In this role, you aren’t just a nurse; you are a teacher and a coordinator who builds lasting relationships with medical providers and families to ensure well-managed, proactive healthcare is received.
What YOU’LL Do:
- Medical Monitoring: Monitor daily documentation for medical needs, identify health trends promptly, and perform health monitoring of documentation to ensure high scores in quality audits.
- Care Coordination: Coordinate with pharmacies and doctors, complete medical appointments, and provide advocacy at critical care consultations.
- Clinical Support: Provide hands-on care when needed, including wound care, ostomy care, catheter management, and monitoring critical care medical needs.
- education & Instruction: Teach medication administration refresher courses, lead clinicals for CMAs and Medication Managers, and provide on-site instruction for staff regarding care plans and medical protocols.
- Administration: Update and maintain care plans and medical protocols while utilizing technology (Therap) for accurate documentation and regulatory compliance.
SCHEDULE: Generally Days, 40hrs/week, OnCall – 1-2 weekdays, every 3rd weekend and holiday rotation

What YOU’LL Need:
- Education: BSN or ASN from an accredited nursing program.

Licensure: Active Iowa nursing license in good standing, valid driver’s license required.
- experience: At least 1 year of general nursing experience, experience supporting individuals with intellectual or developmental disabilities is highly valued.
- Certifications: Current CPR and First Aid (FA) certification.
- Technical Skills: Proficiency with computers and technology is required for daily documentation and task management.
- Physical Requirements: Ability to push, pull, and lift at least 50 lbs
- Soft Skills: Strong critical thinking, time management, and the ability to teach individuals from diverse cultures and learning styles
