Job Description

Activa Home Health, a licensed Medicare certified home health agency, is seeking an experienced Per Diem Field Registered Nurse to provide care throughout Palm Beach County.

The RN will provide the highest quality of patient care by following the individualized Plan of Care, ensuring patient safety, wellbeing, and satisfaction. Bilingual Spanish speaking candidates are strongly preferred but not required.

Required experience, Must Have

• 1 plus year of Home Health experience

• 1 plus year of OASIS experience

• Experience with WellSky Kinnser EMR, required

Job Responsibilities

• Provide safe, effective nursing care based on the individualized Plan of Care

• Communicate promptly with the care team regarding changes in patient condition or emergencies

• Deliver skilled nursing services within scope of practice, including but not limited to,

  Medication administration

  Wound care

  Infusion therapy

  Catheter care

  Transferring and positioning patients

• Complete Start of Care visits using the OASIS assessment tool

• Perform required supervisory visits

• Communicate effectively with patients, families, and interdisciplinary care team members

• Demonstrate empathy, professionalism, attention to detail, and strong follow through

• Anticipate and appropriately escalate or resolve clinical concerns

• Prioritize tasks and respond with a sense of urgency

Required Experience & education

• Active Florida Registered Nurse license

• 1 year of Home Health Medicare experience, required

• OASIS experience required

• Strong wound care and infusion skills

• Excellent communication, collaboration, and customer service skills

• Strong organizational and time management abilities

• Proficiency using electronic health records for documentation and care coordination
