Are you a compassionate Registered Nurse (RN) looking for a flexible schedule and meaningful one-on-one care experience? ComForCare Home Care in Towson, MD is hiring for a Part-time Registered Nurse.

As a Home Health RN, you will provide high-quality nursing care to patients in the comfort of their own homes, ensuring they receive the appropriate medical treatment and assistance they need to manage their health conditions.

Key Responsibilities:

  • Patient Assessments: Perform comprehensive assessments / reassessments of patients’ physical, emotional, and social health, creating individualized care plans based on assessment findings.
  • Care Planning: Collaborate with the interdisciplinary team (physicians, therapists, social workers, etc.) to develop and implement patient care plans.
  • Medical Management: Administer prescribed medications, treatments, and therapies in accordance with medical orders and established protocols.
  • education and Support: Educate patients and their families on disease management, medication regimens, home care practices, and lifestyle modifications.
  • Wound Care: Provide wound care and assist with dressing changes and other specialized treatments as necessary.
  • Patient Monitoring: Regularly monitor patients’ vital signs, condition changes, and progress toward recovery. Report any concerns or significant changes to the physician.
  • Documentation: Maintain accurate and up-to-date patient records in compliance with federal and state regulations. Document patient progress, treatment outcomes, and any changes to care plans.
  • Coordination of Care: Act as a liaison between patients, families, and the healthcare team to ensure continuity of care.
  • Emergency Response: Respond to patient emergencies in a timely manner and provide necessary care until further medical intervention is required.
  • Compliance and Standards: Adhere to the company’s policies, procedures, and relevant regulatory standards to provide safe and effective care.
  • Team Building: Participate and facilitate caregiver in-service programs.

Qualifications:

  • Current Maryland Registered Nurse (RN) license or mutli-state licensure.
  • CPR/First Aid Certification
  • Experience in Home Health, Hospice, and/or Geriatric Nursing preferred
  • Strong assessment and clinical decision-making skills
  • Ability to work evening and weekend shifts, as needed
  • Reliable transportation for home visits.
  • Familiarity with infection control and patient safety procedures

Compensation: $40.00 – $42.00 per hour
