All while understanding that **together** we are even better. **Schedule:** Full Time **Shift** : **Hospitals: 10am-6:30pm Mon-Friday with 2 weekend days/month.** **Field: 11:30am-10pm Mon-Friday every other weekend.** **Every Weekend: Every weekend plus 2 variable days a week. *Weekend Bonus*** **Location:** Covering Seminole, Orange and Osceola Counties. **The role you’ll contribute:** The Admissions Nurse is responsible for responding to and visiting with patients who have been referred for admission to AdventHealth Hospice Care CFL. The Admissions Nurse informs the patients, caregivers, and family members of the hospice services available with the organization and assesses patient for admission to services. In addition, the Admissions Nurse is responsible for visiting current AdventHealth Hospice Care CFL patients who have been admitted to the hospital and communicates patient status with the respective interdisciplinary team. **The value you’ll bring to the team:** + _Provides information and resources available to patients for admittance onto Hospice services._ + _Travels to resident site or hospital room of patient who has been referred to Hospice. Greets patient, caregiver and/or family member(s) and communicates the program and services available with Hospice._ + _Partners with the Account Representative staff member to meet the needs of the patients and hospital staff to gain new referrals._ + _Assesses the patient and family’s emotional condition as to readiness for hospice. Answers questions and responds to concerns to encourage admission onto the Hospice program if conditions are justified._ + _Obtains required signatures and generates handwritten documents required to comply with regulations for reimbursement._ + _Assesses the patient’s physical condition, medical needs, current medications, and needs for special equipment. Determines patient’s primary physician, personal information, and special needs. If patient’s condition requires crisis intervention, psychosocial, chaplain or other special services, notify Hospice of need to assign appropriate team members. Obtain orders and initiate documentation for crisis care if required._ **The expertise and experiences you’ll need to succeed** **:** **Minimum qualifications:** + Associate’s Degree + An equivalent combination of education and experience may be substituted at the discretion of the Administrator + 3 years Registered Nurse experience in home health and/or acute care nursing experience; including 1-year experience in the hospital + RN – Registered Nurse – State Licensure and/or Compact State Licensure + Active American Heart Association BLS + Driver License, Valid and in State Maintain a current, valid Florida Driver’s License in good standing with a safe driving record and a reliable vehicle + Maintain valid personal automobile insurance **Preferred qualifications:** + Bachelor’s Degree in Nursing + Certified Hospice and Palliative Nursing certification This facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
