RN Field Case Manager – Hospice (North Miami-Dade County)

Amedisys Hospice Care

This position isbased in Hialeah, FLand requiresdaily travel throughout North Miami-Dade County. Schedule:Monday-Friday, 8:00 AM – 5:00 PMwith participation in an on-call rotation.

Salary Range:

$80,000-$85,000 annually, plus bonus opportunities.Final compensation may vary based on experience and skills.

Orientation, Onboarding & Training

You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience.

Responsibilities

Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
Records pain/symptom management changes/outcomes as appropriate.
Teaches patients, families, and caregivers appropriate patient care techniques.
Regularly communicates with clinical manager and care team.
Performs on-call responsibilities and on-call services to patients/families as assigned.
Other duties as assigned.

Qualifications

Required:

Full bilingual proficiency in English and Spanish
Current RN licensein the state(s) where you will work
Minimum 1 year of strong clinical RN experiencein an acute healthcare setting
Current CPR certification

Preferred:

Hospice or home health experience
Strong communication and interpersonal skills
Ability to work independently and manage multiple priorities
Comfortable with daily travel between facilities
Proficient in electronic medical record (EMR) systems
