- Deliver high-quality, safe, and effective patient care as part of a collaborative team.
- Promote a positive working culture by encouraging staff participation in service development and improvement.
- Manage inbound referrals and respond appropriately to patient and family queries.
- Update patient records with care plans during MDT meetings.
- Coordinate follow-ups with the clinical team and liaise with hospital teams and practices regarding patient care.
- Process patient discharges and ensure all care-related tasks are completed.
- Report incidents, complaints, and escalations to the Service Manager.
- Perform additional duties in line with NHS policies and legislative requirements as necessary.

- At least 1 year – Administrative skills and robust record keeping
- Ability to effectively communicate and resolve issues over the phone in a professional and courteous manner
- Coordinating and liaising with multiple stakeholders or individuals to meet specified outcomes
Desirable
- Experience of working in Health and Social Care, or similar Public Sector roles
Knowledge and skillsEssential
- Good working knowledge of Microsoft Suite (including Excel, Word, Teams)
- General IT set up and systems
- Excellent written and verbal communication skills
- The ability to plan and organise own workload without direct supervision
- Excellent inter-personal skills and the ability to communicate and work effectively with a wide range of people from different backgrounds at all levels of seniority
- The ability to be innovative, objective as and logical in resolving problems
- The ability to prioritise and balance a range of competing tasks to deliver to agreed timescales
Desirable
- NHS specific IT system experience (SystmOne)
- Understanding of NHS Community Services
- Experience dealing with difficult conversations / complaints
