The Social Worker for Integrated Care Solutions will be able to adapt social work principles and practices to the community setting. This includes, but is not limited to, assisting the community adult client to maximize social, emotional, and cognitive functioning that promote optimum independence and improve quality of life. The Social Worker will conduct comprehensive psychosocial assessments, develop and implement individualized care plans, and provide ongoing support and advocacy for clients. They will collaborate with interdisciplinary teams to ensure holistic care and connect clients with community resources and services. The role involves crisis intervention, case management, and education, aiming to empower clients and their families to achieve their goals and enhance their well-being.Person-and family-centered care (PFCC)Person-and family-centered care (PFCC) is an approach to care that recognizes the importance of family in a patient’s life and the importance of their active involvement in planning and making decisions on health care, services and treatment, and health system reform. This approach to care provides respectful, compassionate, culturally safe, and responsive care that meets the needs, values, beliefs, and preferences of the patient, their family, and others identified as significant to their life from diverse backgrounds and settings.Community Based Position – Per visit rate and travel units, requires Monday to Friday daytime availabilityDuties and Responsibilities
Participates in proactive Health & Safety activities while performing all duties. Is responsible to notify immediate Supervisor of any Health & Safetyrisks or concerns.
