Summary According to social work practice and professional standards, works as a member of the healthcare team to facilitate continuity of care by assessing, planning, coordinating and implementing discharge planning appropriate to the patient#s level of care and post-discharge needs.# Provides supportive counseling, crisis intervention, financial assistance, and information and referral services to both internal and external customers. # Minimum Qualifications Master#s of Social Work degree (MSW). Social Work license in the state of Pennsylvania or license eligible; must receive license within six months of hire. Strong verbal and written communication skills. Strong negotiation skills and the ability to address complex issues with patient/family and staff across multiple departments. Ability to work independently and to gather pertinent patient information. Preferred Qualifications Minimum of 1-2 years# experience in an acute care hospital setting. Fundamental understanding of personal computers and familiarity with the Windows operating system.
