The compensation offered to the candidate selected for the position will depend on several factors, including the candidate’s educational background, skills and professional experience.
Job Overview:
The Social Worker provides direct case management services to patients and their families utilizing an interdisciplinary team approach. This role requires a working knowledge of case management responsibilities including, but not limited to: assessment and documentation, discharge planning, transitions of care, interdisciplinary rounds, regulatory compliance, utilization management, revenue cycle, healthcare financing, inpatient capacity, patient flow, and healthcare related social needs social determinants of health. This role requires collaboration with the interdisciplinary team, including physicians, clinical nurses, and others as needed. The Social Worker will actively participate in initiatives that improve key performance indicators such as length of stay, throughput that aligns with RWJBH System Goals and ensures continuity of care. The Social Worker will provide clinical social work interventions, including supportive counseling, crisis intervention, and psychotherapeutic approaches that address psychosocial distress arising from illness, grief and loss, trauma, or other major life events,
Qualifications:
Required:
Graduation from an accredited graduate school of social work with a Master’s in Social Work MSW degree,
State licensure required by the NJ State Board of Social Work examiners: Licensed Social worker (LSW) or Licensed Clinical Social Worker (LCSW)
Preferred:
Case Management certification is preferred.
Advanced training or certification in clinical social work practice: trauma-informed care, family therapy, and/or palliative care is valued.
One or more years of experience in an acute hospital setting is preferred.
