A Brief Overview

Children’s Hospital of Philadelphia is seeking a Wound Ostomy Program Clinical Nurse Coordinator. This position will be Monday-Friday and the shift will be based off of the operational need.

In collaboration with the Wound, Ostomy, Continence Advanced Practice Nurse and the Director of Advanced Practice Nursing, the Wound/Ostomy Program Clinical Nurse Coordinator will support the clinical care, program development and quality improvement activities related to patients with wound, ostomy, and continence concerns.
What you will do

• Clinical Excellence:
• Provides nursing expertise regarding skin, wound and ostomy care needs to inpatients:
• In partnership with Wound, Ostomy Continence (WOC) Team, provides direct care to patients with wound, ostomy or other skin care needs. Performs patient assessment, institutes a plan of care, and works with the staff, patient, and family to promote positive patient outcomes and transition from hospital to home or lower level of care.
• In partnership with Wound, Ostomy Continence (WOC) team, Skin Care Champions, and Multidisciplinary Wound Care Forum, identifies key clinical practice changes required to enhance care of patients with pressure injury and pressure injury risk, skin and wound care problems, and ostomy care
• In partnership with WOC team, leads execution of all new practice changes related to pressure injury and wound & ostomy care in the clinical area, utilizing evidenced-based approach.
• Makes recommendations for culture change required to improve documentation related to wound and ostomy care, including documentation of pressure injuries, pressure and other skin injuries present on admission, pressure injury prevention interventions, and pressure injury risk assessment, working collaboratively with both nursing and physician provider groups
• Conducts rounds for purposes of identifying high risk patients and patients requiring WOC team consults or nursing notifications, and providing follow-up care for patients with complex wound/ostomy needs.
• Develops own expertise in pressure injury staging, wound and ostomy care of pediatric patients, and begin to provide training for unit staff
• Participates in RCA/ACA regarding hospital acquired pressure injuries or other hospital acquired skin injury
• Participate in quarterly Pressure Injury surveys and, in collaboration with WOC team, organize and analyze pressure injury data from these surveys and from monthly KAPS reports
• Provides feedback, encouragement and enthusiasm for improvement work at the unit level in terms of pressure injury and risk assessment documentation and documentation of wound and ostomy care.
• Professional Engagement:
• Develops additional leadership skills to influence practice at the unit/program level:
• Understand the barriers that may prevent 100% adoption of wounds and ostomy related standards, pressure injury prevention standards, documentation of care and prevention practices, and problem solve to promote behavior change. Examples include: what may impede documentation of risk assessment on admission, documentation of pressure injury present on admission, and ongoing documentation of pressure injury interventions (turning, etc.)
• Advancing Practice:
• Gains proficiency in helping staff adopt clinical practices which enhance wound and ostomy care, and pressure injury prevention:
• Engage and collaborate with unit based Skin Champions in conducting evidence-based reviews topics of related to pressure injury, wound and /or ostomy care
• Lifelong Learning & Innovation:
• Increases skill in evidence based reviews regarding wound and ostomy care :
• Complete education regarding wound and ostomy care and pressure injury staging and seek out education specific to these concerns in the pediatric patient.
• Develop and/or contribute to the development of innovative wound and ostomy care, including participating in new product trials
• Participate in professional development activities including: journal club, professional presentations, nursing research and EBP.
• Fiscal Stewardship:
• Advances own understanding of fiscal stewardship in pressure injury prevention, wound and ostomy care:
• Increases knowledge related to Lean principles and applies these principles to a unit/program regarding use of products related to pressure injury prevention (Specialty beds, Z flo), dressings and ostomy products

Education Qualifications

• Bachelor’s Degree Nursing Required
• Master’s Degree Nursing Preferred

Experience Qualifications

• At least five (5) years nursing experience. Required
• WOC experience Preferred

• Demonstrated effective communication skills with Physicians, Nurse Managers, Senior Leadership and all employees at all levels. (Required proficiency)

Licenses and Certifications

• Registered Nurse (Pennsylvania) – Pennsylvania State Licensing Board – upon hire – Required
• Certified Wound Ostomy Continence Nurse – Wound Ostomy Continence Nursing Certification Board – within 24 months – Required

SALARY RANGE:

$98,820.00 – $126,000.00 Annually

Salary ranges are shown for full-time jobs. If you’re working part-time, your pay will be adjusted accordingly.
