HouseCalls is a unique, outcomes-focused program that delivers healthcare directly in members’ homes. As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you’ll perform annual in-home health assessments for Medicare Advantage and other eligible health plan members. This field-based, non-prescribing role is designed to support preventive care, close care gaps, and collaborate with primary care providers to improve overall health outcomes.

Primary Responsibilities:

  • Complete comprehensive in-home health evaluations, including:
    • Medical history review and medication reconciliation
    • Vital signs assessment and physical examination
    • Evidence-based screenings and point-of-care testing when appropriate
  • Identify and document conditions that support care planning and disease management
  • Communicate assessment findings to primary care providers to help address care gaps
  • Recognize and appropriately respond to urgent or emergent health concerns
  • Educate members on chronic conditions, medications, and treatment adherence
  • Assess social determinants of health and connect members with appropriate resources and services
