What You’ll Do:

• Deliver skilled physical therapy services in patients’ homes

• Assess mobility, strength, balance, and functional limitations

• Develop and implement individualized treatment plans

• Educate patients and caregivers on home exercise programs and safe mobility strategies

• Monitor progress, document outcomes, and adjust care plans as needed

• Collaborate with an interdisciplinary care team to ensure excellent patient outcomes

What You’ll Need:

• Active Florida Physical Therapist (PT) license

• Degree from an accredited Physical Therapy program

• Reliable transportation and valid driver’s license

• Current CPR certification

• Strong communication and time-management skills

• Home health experience (preferred)
