As the RN Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for your home health patients making necessary changes to maintain quality of care delivered. This includes:
Making the initial nursing evaluation and creating an appropriate and patient-specific plan of care
Identifying the patient’s/family’s physical, psychosocial, and environmental needs and tailoring each care plan appropriately
Completing recertifications or resuming care for eligible patients
Initiating and coordinating the plan of care while maintaining accurate and relevant clinical notes regarding the patient’s condition
Documenting problems, appropriate goals, interventions, and patient/family response to skilled nursing care
Collaborating with the patient/family, attending physician, and other members of the IDG in providing patient and family care
